4. If you have a website, you MUST place a code on your site where it can easily be seen, such as a fanlisting, membership, or main page and link it back here BEFORE submitting your information. If the code is not on your site at anytime or if it's too hard to find, I will delete your website information.
5. You MUST fill out the form correctly and completely.
